After- Hospital Care
A client who has been discharged from the hospital but is not fully recovered or has a condition that needs monitoring, for example, surgery, heart attack, stroke, etc.

Chronic Condition Care
A person who has a chronic condition and requires care until he/she has stabilized (e.g., heart/lung diseases, diabetes, etc.

Wellness Care
Someone recently diagnosed with a disease that needs education and/or teaching about the illness and help coping with, for example, high blood pressure, etc

End Of Life Care
Someone who is in the final or end stages of an incurable or terminal disease.

Repeat Hospitalization
A person who is being hospitalized repeatedly or is experiencing prolonged hospital stays.

Elderly Home Care
A family who is caring for an elderly, disabled, or family member who needs education or additional support.

In Bed Care
Anyone who is not capable of functioning independently on their own needs helps with activities of daily living and requires watchful oversight/supervision.

Personal Care
Medically frail and/or compromised (physically weak) clients.
